Licensed Practical Nurse

Location: Syracuse, NY

JOB SUMMARY:

Assists in providing leadership, supervision, and support to staff in performing quality resident care in compliance with the philosophies, objectives, and established policies and procedures of the department and facility.

 

ESSENTIAL JOB FUNCTIONS:

  1. Receives report on residents from the Charge Nurse going off duty and gives a complete report on residents to oncoming nurse prior to leaving duty.
  2. Reviews the condition of residents with other nurses on the unit and communicates pertinent information to other staff members involved with resident care.
  1. Counts controlled substances/narcotics with previous shift nursing staff when arriving on duty. Ensures that all medications are properly stored, counted and locked.
  2. Evaluates residents and refers information to the Nurse Manager, House Charge Nurse, and/or Supervisor when necessary. Assists in scheduling appointments for referrals to clinics and other doctors.
  1. Participates in meetings and committees as requested.
  1. Willingly assists in supervising the work performance of the nursing assistants and

assists when necessary.

  1. Evaluates and performs nursing care of all residents per physician’s orders and in

compliance with facility policies, procedures ,and care plans , including but not

limited to:

  1. Medication administration
  2. Wound/Skin Care dressing changes/treatment applications
  3. GT/NGT feeding
  4. Vaginal douches, enemas, suppositories
  5. Suctioning and tracheostomy care.
  6. Colostomy care and irrigations.
  7. Foley catheter changes and irrigations.
  8. Oxygen therapy, monitoring O2 Sat.
  9. Assists in emergency situations.
  1. Performs documentation in compliance with facility policies and procedures including, but not limited to: Charts, medications, treatments, procedures, new orders, therapeutic effects/pain, psychotropic medication changes, and resident tolerance
  2. Admissions, discharges, transfers and physician visits.
  3. Documents all significant or acute care issues in the Nurse Progress note with follow up to MD and family.
  4. Nurses notes, summaries, progress notes, pain management, weekly summary note, behavior charting.
  5. Documents all resident/family education.
  6. Completes 24 hour shift reports, census reports, and signs completed
  7. Laboratory, x-ray results
  8. Infection Control log and employee’s “call in” book
  9. Supervisor’s report
  10. Skin/wound charting (treatments, changes, and therapeutic effects)
  11. Keep charts current and in compliance with regulations.
  12. Responsible for charting I & O’s when ordered.
  13. Assists in charting admission notes and 7 day follow-up and 5 day follow upon readmission.
  14. Accidents, incidents, expirations
  15. Oversees all CNA documentation for completion and accuracy every shift.
  16. All nursing staff is accountable for full completion of all documentation prior to the end of their shift. All forms are to be filled in for accuracy; no holes are to be left on any forms
  17. Helps educate the resident and family in all aspects of their health condition and treatment.
  18. In order to provide the highest quality care to our patients and residents, individuals may be required to work hours beyond those normally scheduled. Work schedules may be subject to change based on the needs of the department.
  19. Acts as a role model while maintaining a professional, ethical and respectfully values while maintaining resident/staff confidentiality.
  1. Other duties as assigned.

 

WORK ENVIRONMENT:

Position requires frequent lifting, pushing, pulling and moving of over 50 pounds. The employee must walk, stoop, and kneel, reach and handle, talk and hear, and see and move intermittently during working hours. The work is inside and outside and involves a variety of duties.

Must treat resident information as a confidential material

Follows nursing home policies and procedures

Respects residents’ rights and dignity

Reports any suspected resident abuse as required by government regulations and the nursing home policy.

Attend all required Mandatory in-services and seminars

Required to follow established policies in regards to tardiness, attendance, and dress code

Serves as a positive ambassador to the Health Center, residents, staff, visitors and the community

 

QUALIFICATION STANDARDS:

Must possess a current license (or eligible for license) to practice as LPN in the State of New York.

 

SALARY PER HOUR

New Grads: $21.50

Experienced: $26.50

Evenings: $27.75
