Liaison

JOB SUMMARY:

 

Coordinates and facilitates admissions for the nursing home.

 

ESSENTIAL JOB FUNCTIONS:

 

1. Maintain relationships with all referral sources including but not limited to Hospitals, VA facilities, CASA, Care Management, Social Workers, Nursing, Physicians, and home type referrals.

 

2. Review incoming data from referral sources including but not limited to medical chart, ECIN data information, physician’s view, medical faxes and CASA.

 

3. Reviews prospective resident medication list with a subsequent cost analysis.

 

4. Determines matched bed availability based on diagnosis/prognosis and ability to meet determined need as documented by Nursing, Physician, PT and OT.

 

5. Determines resident eligibility and coverage and obtains pre-authorizations and pre-certifications as required by individual insurance carriers.

 

6. Negotiate private insurance contracts as indicated.

 

7. Remains current in legislative, regulatory, and professional practice issues.

 

8. Ensure appropriate admissions paperwork, referral sources, and other admissions data are entered into the computer system.

 

9. Greet new admissions and facilitate a smooth transition into GHHCC obtaining all necessary signatures from resident/responsible party in a timely manner.

 

10. Ensure in-house transfers are coordinated with nursing, Social Services department and Medical Records.

 

11. Acts as a patient advocate.

 

12. Provides data collection as requested.

 

 

 

 

 

WORK ENVIRONMENT:

 

This position involves a variety of duties and responsibilities. The work is inside as well as outside. The employee must be people oriented. The employee sits, stands, and walks during working hours. The employee must also talk, hear and see.

 

 

ACCOUNTABILITIES:

 

Is responsible and accountable to Administrator.

 

Maintain confidentiality on all resident information.

 

Follows nursing home policies and procedures.

 

Respects residents’ rights and dignity.

 

Reports any suspected resident abuse as required by government regulations and the nursing home policies.

 

Attend all required in-services and seminars.

 

Required to follow established policies in regard to tardiness, attendance, and dress code.

 

Serves as a positive ambassador to the Centers residents, visitors, and the community.

 

 

QUALIFICATION STANDARDS:

 

At least two years of working in the local medical community.
