WORK AREA: Resident's room, units, and office area

JOB SUMMARY:

Plan, develop, implement and evaluate social services programs and activities. Ensure quality and safe delivery of services that reflect the nursing home's philosophy and standard of care.

ESSENTIAL JOB FUNCTIONS:

1. Responsible for functions of the social services department and ensures that all social service functions are carried out through appropriate policies and procedures in accordance with state and federal regulations.

2. Provide on-going supervision of social work staff as they carry out their daily social work functions and conduct periodic evaluations of their performance as are required by the facility.

3. Provide consultation and education for the staff of the facility regarding residents' social needs and residents' rights.

4. Responsible for coordinating weekly interdisciplinary resident care conferences

and comprehensive interdisciplinary discharge planning, as needed. Acts as the

residents' advocate in relationships with the nursing home, family and outside agencies.

5. Maintains relationships with psychologists; manages psychological services and referrals.

6. Serves as a resident advocate throughout their stay in the facility to ensure resident rights, advance directives, end of life and counseling services are provided to meet psychosocial needs.

7. Handle resident and family concerns and are involved in the resident/family counsels.

WORK ENVIRONMENT:

Works in office area as well as throughout the nursing home facility. The employee must move, stoop, kneel, reach, handle, talk, hear, and see. Position requires occasional pushing, pulling, and lifting of 10-25 pounds.

ACCOUNTABILITIES:

Provide support, guidance and counseling to residents and families so that each resident is guided to help himself or herself to greater independence and a more meaningful life.

Acts as the residents' advocate in relationships with the nursing home, family, and outside agencies.

Attend daily IDT meetings.

Coordinates Weekly UR & Medicare Non-Covered letters.

Assist resident/family in planning their financial and legal needs while maintaining communication with the Business Office with Insurance referrals & recertifications.

Accurate and timely completion of all assessments, MDSís, and care plans to ensure effective links with social supports, physical care and the environment for the residentís needs.

Assist in reducing chemical and physical restraints by identifying why residentís act as they do, effective communication and identifying needs to be met.

Other duties as assigned by the Administrator.

Must treat resident information as confidential material.

Follows nursing home policies and procedures.

Respects residents' rights and dignity.

Reports any suspected resident abuse as required by government regulations and the nursing home policies.

Attend all required inservices and seminars.

Mandatory inservices must be met within time required.

Required to follow established policies in regard to tardiness, attendance, and dress code.

Services as a positive ambassador to the†Center, residents, staff, visitors, and the community.

QUALIFICATION STANDARDS:

License in Social Work required.

Long-term care experience is preferred.
